What you need for this examination

Please bring this in:

  • VACCINATION RECORDS (any which you may have).
  • Quantiferon gold test (if you have this).
  • Proof of Covid vaccine, must be less than 1 year old.
  • Proof of CURRENT year’s flu vaccine (If your examination is between September and April).
  • Tests for RPR and Gonorrhea (less than one year old) if you have them.
  • Proof of any immunization you have received with in the last 10 years (some of this may help you).
  • GOVERNMENT ISSUED PHOTO ID.
  • RESULTS OF ANY OTHER TB TESTING PERFORMED WITHING THE LAST 365 DAYS.
  • MEDICAL RECORDS C/O ANY PSYCHIATRIC OR SUBSTANCE USE TREATMENT.

 

Immigration Examination
